STEP 2 : તમારો મોબાઈલ નંબર અને કેપ્ચા કોડ દાખલ કરો અને 'OTP જનરેટ કરો' પર ક્લિક કરો

STEP 3 : પછી તમારું રાજ્ય પસંદ કરો અને નામ/HHD નંબર/રેશન કાર્ડ નંબર/મોબાઈલ નંબર દ્વારા શોધો

STEP 4 : શોધ પરિણામોના આધારે, તમે ચકાસી શકો છો કે તમારું કુટુંબ PMJAY હેઠળ આવરી લેવામાં આવ્યું છે કે કેમ

STEP 5 : ત્યાર બાદ તમને 24 અંક નો HHID નંબર જોવા મળશે. જે તમારે સાચવી ને રાખવો. આયુષ્માન કાર્ડ કઢાવતી વખતે જરૂર પડશે.
વૈકલ્પિક રીતે, તમે PMJAY માટે લાયક છો કે નહીં તે જાણવા માટે, તમે કોઈપણ હોસ્પિટલ નો સંપર્ક કરી શકો છો જે પ્રધાન મંત્રી જન આરોગ્ય યોજના હેઠળ ચાલતી હોય અથવા આયુષ્માન ભારત યોજના કોલ સેન્ટર નંબર: 14555 અથવા 1800-111-565 પર ડાયલ કરી શકો છો.
જે HHID તમને મળ્યો એ લઈ ને તમે નજીક ની હોસ્પિટલ માં જઈ ને આયુષમાન કાર્ડ બનાવી શકો છે અથવા નજીક નું CSC સેન્ટર પર જઈ ને પણ આયુષ્માન ભારત કાર્ડ બનાવી શકો છો.
નીચે આપેલ જરૂરી ડોક્યુમેન્ટ્સ સાથે લઈ જવાના રહેશે.

આયુષ્માન કાર્ડ ના ફાયદા (Benefits of Ayushman card)યોજના હેઠળ ગરીબ લોકો 5 લાખ સુધીની મફત સારવાર મેળવી શકે છે.
દેશના નાગરિકો આયુષ્માન ભારત કાર્ડ દ્વારા તેમની સારવાર માટે સરકારી અને ખાનગી હોસ્પિટલોમાં જઈ શકે છે.
50 કરોડથી વધુ અરજદારો આ યોજનાનો લાભ લઈ રહ્યા છે.
